Advance directives are typically written documents that convey an individual’s intentions for the provision, withholding or withdrawal of life-sustaining procedures, and can specify the conditions for a wide range of future health care decisions. Hospitalist and Intensivist Services

At Holy Cross Hospital and Holy Cross Germantown Hospital, care for most medical patients is provided by specialists in hospital-based medicine. These physicians are referred to as hospitalists or in the Intensive Care Units as intensivists. They coordinate patient care among all members of the hospital care team (including community physicians).

Palliative Care Consultation Services

Holy Cross Hospital and Holy Cross Germantown Hospital offer comprehensive care for patients with serious illnesses. The palliative care team works closely with the attending physician and nursing staff to provide expert pain and symptom control, health education, advance care planning, and coordination of services for patients faced with a serious illness and their families.
