Getting Started

MyChart provides personalized and secure online access to your health records and helps you manage and receive health information. With MyChart, you can:

  • View test results.
  • Request prescription refills.
  • View bills and set up payment options.
  • Review after-visit summaries and instructions from recent office visits or hospital stays.
  • Schedule and view future appointments, including virtual video visits.
  • Communicate securely with your doctor and care team.

Primary care patients of Holy Cross Health Centers and Holy Cross Health Partners will also be able to:

  • Confirm your appointment, complete pre-visit paperwork and electronically check in before your visit.
  • Be added to a waitlist to be notified of earlier appointment openings.

No. MyChart is a free service for our patients.

At your next doctor’s visit, request a MyChart instant activation link, valid for 14 days, via text or email. You can also use an activation code found on your After-Visit Summary sheet or Billing Correspondence. To register, go to mychart.trinity-health.org and enter the activation code. 

Trinity Health also offers self-service sign-up for those without an activation code. Download this step-by-step guide to get started (also available in Spanish). Please note, enter your personal information to match what we have in our system, exactly as it appears in our records.

Your computer or mobile device (iOS or Android) must connect to the Internet, and you will need an up-to-date browser such as Google Chrome, Microsoft Edge, Safari or Firefox. Use of Internet Explorer is not recommended.

Patients aged 12 years or older can create a MyChart account. Individuals aged 12 to 17 must have parental or guardian consent or be an emancipated minor.

If your MyChart account is with a different healthcare system or provider, you will need to create a new account specifically for Holy Cross Health. However, you can link your accounts with other organizations that use MyChart.

Yes, you may obtain proxy access to a family member’s records, depending on your relationship. Proxy access allows a parent, guardian or power of attorney to log in to his/her personal MyChart account and connect to information regarding his or her family member.

  • To request access, download and complete the MyChart Patient Proxy Consent Form (also available in Spanish).
  • Primary care patients of Holy Cross Health Centers and Holy Cross Health Partners must submit the Proxy Consent Form in person.

Using MyChart

MyChart provides a summary of your health information based on your doctor’s entries in your portal. You can view future and past appointments, test results, current health issues, your medications, allergies, immunizations, and medical history. Note: Some features are available only to primary care patients of Holy Cross Health Centers and Holy Cross Health Partners.

MyChart provides an easy way to ask your Holy Cross Health primary care doctor basic, non-urgent questions via secure electronic messaging. You can also request medication refills without having to call or visit the office. Primary care patients also have access to convenient health reminders for preventive screenings such as mammograms or prostate exams, helping you be a more active partner in maintaining and improving your health.

These apps give you the ability to view some of your medical record (or someone else's records, where you have proxy rights) on an app or website other than your patient portal.

How do I connect my medical record with an app? 

To access your record through one of these apps (or someone else's where you have proxy rights) you must first create a patient portal account that is linked to Holy Cross Health. If you do not have access, please sign up by creating an account by using the Patient Portal page.

Billing 

Yes. With MyChart Bill Pay, you can make secure payments. You will receive a payment confirmation message in the MyChart message center. You do not need a MyChart account to pay your bill. You can pay as a guest from the MyChart page.

Please note: Your bill may not include fees from your surgeon, anesthesiologist or professional services billed by other physicians and providers. Typically, you will be billed separately for those services.

No, there is no charge to use the MyChart Bill Pay functionality.

Previous Patient Portals

Your provider will have access to your entire medical record. If you have a current patient portal account with Holy Cross Health, some of your historical health information has been transferred to your MyChart account over time and will be visible to you.

Requesting Health Records

You may request medical records by completing and submitting an Authorization for the Release of Medical Information form (also available in Spanish). Completed forms can be emailed to HIMReleaseofInfo@holycrosshealth.org. Please note there may be a fee for providing copies of your medical record.

Privacy

Your name and email address will be treated with the same care and privacy as your health records and will never be sold or provided to anyone else. MyChart complies fully with federal and state laws related to privacy.
