Getting Started

MyChart offers you personalized and secure online access to your medical records. It securely enables you to use the internet to help manage and receive information about your health. With MyChart, you can:

  • View select information in your health record.
  • View test results.
  • Pay your bills online.
  • Access trusted health information resources.
  • Download the Trinity Health MyChart app to keep track of your health using your smartphone and/or tablet. Note: some features are not available when accessing your account via the app.

Primary care patients of Holy Cross Health Centers and Holy Cross Health Partners will also be able to:

  • Schedule appointments and view future appointments including video visits. 
  • Request prescription refills.
  • Communicate privately and securely with your care team.
  • Confirm your appointment and electronically check-in to complete pre-visit paperwork online before your appointment.
  • Be added to a waitlist to be notified of earlier appointment openings.
  • Get care through video visits.

No. MyChart is a free service offered to our patients.

We encourage you to sign up for your MyChart account at your next doctor’s visit. At your visit, you will receive a MyChart instant activation link, valid for 14 days, via text or email. Another way to register is using an activation code found on your After-Visit Summary sheet or Billing Correspondence. Go to mychart.trinity-health.org and register using the activation code.

Also, at any time, Trinity Health offers self-service sign-up for those who do not have an activation code. Download this step-by-step guide to get started (also available in Spanish). Please note, the personal information entered must match the information we have in our system exactly as it appears.

If you encounter difficulties, please call the MyChart Support Line at 301-754-7363. You can also sign up for MyChart online or at your next appointment.

You will need a computer or mobile device (IOS or Android) that can connect to the Internet and an up-to-date browser, such as Google Chrome, Microsoft Edge, Safari, or Firefox. Use of Internet Explorer is not recommended.

You must be 12 or older to use MyChart. MyChart users 12 to 17 years of age must have parental or guardian consent or be an emancipated minor.

If your MyChart account is with a different health care system or provider, you will need to create a new account specific to Holy Cross Health. MyChart is utilized by many health care providers and systems throughout the country. Fortunately, if you have an active patient portal account at another organization that uses MyChart, you will be able to link your accounts.

Using MyChart

MyChart provides a summary of your health information based on your doctor's entries in your patient portal. You can view your future and past appointments, test results, current health issues, medications you are taking, allergies, immunizations, and medical history. Note: some features are only available to primary care patients of Holy Cross Health Centers and Holy Cross Health Partners.

MyChart provides an easy way to ask your Holy Cross Health primary care doctor basic, non-urgent questions using secure electronic messaging and request medication refills, without having to call or visit the practice. Primary care patients will also have access to convenient health reminders, such as when you are due for preventive tests like a mammogram or prostate exam to help you be a more active partner in maintaining and improving your health.

Billing

Yes, you may access some family members' records, depending on your relationship. Proxy access allows a parent, guardian, or power of attorney to log in to their personal MyChart account, and then connect to information regarding their family member. Download and complete the MyChart Patient Proxy Consent Form (also available in Spanish) to request access to this convenient service. Primary care patients of our Holy Cross Health Centers and Holy Cross Health Partners practices will be required to submit their forms in person. Hospital patients can email or fax their forms to HIMReleaseofInfo@holycrosshealth.org or 301-754-7175 for Holy Cross Hospital or 301-557-5551 for Holy Cross Germantown Hospital.

Yes! With MyChart Bill Pay, you’ll have access to make secure payments through the MyChart application. You’ll receive a confirmation message in your MyChart message center. You do not have to have a MyChart account to pay your bill. You can pay as a guest from the MyChart page.

Please note your bill may not include all fees from your surgeon, anesthesiologist, or other professional services billed by your physician(s) and other professional providers. Typically, you will be billed separately for these professional services.

No, there is no charge to use our MyChart Bill Pay functionality.

Previous Patient Portals

Your provider will have access to your entire medical record. If you currently have a patient portal account with our health system, some of your historical health information will be transferred over a period of time to your MyChart account and be visible to you.

Requesting Health Record Copies

Holy Cross Health patients may request medical records by filling out and submitting an Authorization for the Release of Medical Information form (also available in Spanish). Completed forms can be emailed to HIMReleaseofInfo@holycrosshealth.org. Please note there may be a fee for providing copies of the medical record.

Privacy

Your name and email address will be treated with the same care and privacy given your health records and will never be sold or provided to anyone else. MyChart complies fully with federal and state laws related to privacy.

The MyChart Support Line, 301-754-7363, is available to answer your questions and assist with access seven days a week, from 7 a.m. to 7 p.m. Eastern Time (including holidays).
