Disease Management Services

At Disease Management Services, we understand that managing chronic conditions can be challenging. That’s why we offer a variety of evidence-based workshops for self-management of health issues like diabetes, hypertension, cancer, COPD, and more.

Our mission goes beyond just education—we strive to transform lives. Our classes provide you with tools to address management of chronic illnesses through individual action plans and the opportunity to share and learn from others.
