Have you been touched in a positive or inspiring way by a Holy Cross Health team member? Would you like to offer words of encouragement and gratitude to the brave men and women serving in our hospitals and clinics for the health of our community during the COVID-19 pandemic? You can support our frontline heroes by sharing uplifting stories, photos and videos using the form below.

We encourage your creativity! You may submit photos with hand-held signs delivering a written message, or you may speak your message in a short video (ideally :15 or less). Please follow these guidelines—we will review the submissions and share as many as we can with our frontline colleagues, and possibly through our website and social media channels.

• Maintain safe social distancing practices at all times.
• If the person in your photo/video is under age 18, a parent will need to authorize the submission by completing the form below.
• If there is more than one person in the photo/video, each person seen or heard will need to complete their own authorization form below.
