Health Education
Health education topics include diabetes, nutrition, high blood pressure, cardiovascular wellness, and weight control. The centers also host health fairs for our multicultural community.
Physical Exams and Lab Work
Our medical staff complete thorough assessments and will order lab work as needed through on-site blood work or through a specialized referral at Holy Cross Hospital or Holy Cross Germantown Hospital.
Diabetes and Cardiovascular Management
We specialize in the prevention and management of diabetes and cardiovascular disease. Our doctors and nurses provide assessment, education, and a specialized care plan for our patients.
Pharmacy
We have a small on-site pharmacy for Montgomery County residents. If we are unable to provide medications from our pharmacy, we can assist patients with obtaining no-cost and low-cost medications.
Assessments and Referrals
A trained licensed clinical social worker and patient navigator are part of our comprehensive staff available to assist patients with mental health assessment and social service referrals.
