Eligibility Information

Holy Cross Health Centers provide services to uninsured children and adults for an adjusted fee based on financial need, and to community members enrolled in Maryland Medicaid/Maryland Children's Health Program.

When you arrive for your first appointment, you will complete paperwork about your health history and meet with our financial counselor, who will verify insurance or discuss payment for services.

In order for us to approve discounted pricing, uninsured patients must submit the following documents to verify eligibility:

• Photo identification
• Last year's tax forms
• Last two paycheck stubs
• Last two bank statements
• Mortgage or lease papers

Payment is due at the time of service and may be made by cash, check or major credit card.

For Medicaid Recipients in the HealthChoice Program

Holy Cross Health Centers only accepts patients enrolled in the Medicaid HealthChoice program through Maryland Physicians Care.
