Transportation To and From the Hospital
We encourage you to ask family or friends to schedule their day to be available to support you after your procedure. In the event you do not have someone available to take you home, you do have options. Here is a list of contacts that provide transportation and/or companionship for a fee:

Resources Provided in the Home
Home Care and Hospice
Holy Cross Home Care and Hospice Agency provides Medicare-certified home health and hospice services for patients who need care at home regardless of whether they have had prior hospitalization. Home health services can include skilled nursing, home health aides, medical social work, and physical, occupational and speech therapy. Hospice services for those who are terminally ill can include chaplains, volunteers, respite care and bereavement counseling.
The Geriatric House Calls by Nurse Practitioners Program enables older adults to receive primary medical care at home, delivered by a nurse practitioner specially trained and certified to provide comprehensive medical care, including examining and diagnosing patients, arranging for lab and other diagnostic services, and prescribing medications.
Private Home Services
Holy Cross Private Home Services, Inc., provides home health aide services to clients in Montgomery and Prince George's counties. Aides, under the direction of a registered nurse, provide personal and custodial care, live-in services and assistance for new mothers. Clients have access to an on-call coordinator 24 hours a day.
Resources Available Outside the Home
Caregiver Resource Center
Holy Cross Hospital's Caregiver Resource Center attempts to ease the burdens of family caregivers by providing support groups, educational programs and other resources to those caring for a family member. The Caregiver Resource Center is located one block from the hospital at 9805 Dameron Drive.
Holy Cross Hospital Senior Source
A partnership between Holy Cross Hospital and Montgomery County, the Holy Cross Hospital Senior Source provides physical activity, education and information on health and wellness for adults 55 years of age and older. The center is located at 8580 Second Avenue in downtown Silver Spring.
Medical Adult Day Center
Holy Cross Hospital Medical Adult Day Center is a caring, compassionate and positive environment where older and medically disabled adults can participate in recreational activities and receive health care. The center is located one block from the hospital and welcomes new applicants ages 18 to 100+ on an ongoing basis.
Outpatient Nutrition Counseling
Outpatient nutrition counseling is available to patients who need assistance in making healthier food choices after they are discharged from the hospital. Comprehensive nutrition assessments are offered for people with cancer, diabetes, heart disease and nutrition-related conditions.

Health and Wellness Education
Throughout the year the Community Health department conducts classes, seminars and other events that help you manage and improve your health. Topics range from fitness and movement, to health and wellness, to caregiver resources, with a special spotlight on classes of interest to seniors.
