Neurosurgery Program
What You Need to Know
The Holy Cross Hospital neurosurgery clinic is located
on the hospital’s second floor. At the main lobby take the elevators
on the right to the second floor. The clinic is located on the second
floor across from the elevators.
If you are new to the neurosurgery clinic,
please arrive about 30 minutes before your appointment to fill out
the appropriate forms. You may print and fill out the preregistration
form (coming soon) in advance. Remember to bring
the filled-out form with you.
Please make sure you have the following
items with you at your appointment:
- referral letter from your physician
- your most recent radiographs/films (MRI, CT, X-ray). Please bring
the actual films (not just the report) so that we can evaluate you.
Without these films, we will reschedule your visit until you can
obtain them. If your radiographs were done at Holy Cross Hospital,
you do not need to bring a copy with you.
