Holy Cross Home Care
Holy Cross Home Care has been providing comprehensive, community-based home health services since 1967. After your joint replacement, you can receive homecare services, even if your health care needs are great.
We offer an interdisciplinary team of skilled professionals to help you reach your highest level of independence and function. We can provide physical and occupational therapy in your home based on your specific individualized treatment plan.
Physical therapy
Physical therapy will continue with functional mobility to include
walking in your home setting, getting in and out of your bed, and
stair climbing so that you can get in and out of your home easily.
Your specific home exercise protocol will be progressed in the
home along with the assessment of any durable medical equipment needs.
Occupational therapy
Occupational therapy will continue with bathing or showering in your
bathroom as well as dressing activities at home.
Registered nurses
Registered nurses are also available should a medical need arise
during your rehabilitation at home such as pain management or wound
care.
Medical social workers
Medical social workers can provide information regarding community
resources.
Certified home health aides
Certified home health aides are also available for assistance with
activities of daily living.
