Holy Cross Hospice
Taking care of someone who is facing a life-threatening illness can be
overwhelming. Holy Cross Hospice can provide an interdisciplinary team
of skilled professionals to help you.
After an initial assessment, the team, in conjunction with you and your
physician, will create a plan tailored to meet the needs of you and your
family. We provide the skilled care and compassion we know you need, every
step of the way.
Holy Cross Hospice emphasizes the importance of
maintaining patients in their home environment during the terminal stages
of an illness. However, our team of caregivers can provide support in
many institutional settings as well. Anyone with a terminal or life-threatening
illness may be cared for by Holy Cross Hospice.
Expert, Interdisciplinary Care
We have assembled a team of experts across
a broad range of disciplines to assess and provide support services to you
and your family. Services include:
- Skilled nursing: Registered nurses and licensed
practical nurses assess, evaluate, manage and provide instruction
for a wide range of conditions and symptoms. All of our nurses have
special training in pain control and symptom management.
- Social work: Medical social workers provide planning,
counseling and support for you and your family in dealing with the
social, emotional, financial and practical needs you may encounter.
- Personal assistance: Certified home health aides provide
personal care assistance such as bathing, hair and nail care, bed-making,
toileting, light meal preparation and light laundry.
- Spiritual support: Pastoral Care representatives
provide spiritual support to our hospice patients and their families.
Spiritual support is available for all religious affiliations.
- General support: Volunteers, specially trained
in hospice, work with the professional staff to provide practical
assistance, respite and support to patients and families.
- Bereavement support: Grief
support for adults and children is available both
on an individual and group basis to anyone in the community experiencing
grief due to the loss of someone they love. We also offer community
counseling services in school or the workplace after a traumatic
event.
We accept Medicare and Medicaid as well as most other insurance plans.
We will help determine your coverage level before services start.
Our Program is state-licensed, Medicare-certified
and Joint Commission accredited.

A Trinity Continuing Care Services Facility
The Bayles Center at Holy Cross Rehabilitation
and Nursing Center is one of several facilities available to patients
whose care needs can not be provided at home.
