To receive your free cards fill out the following form. When you receive the cards, take a few minutes to fill in your health information. Keep one card in your wallet, and give the other to a family member or caregiver to keep on hand - or post it in a convenient place in your home. Take it with you to every doctor's visit, and update the information as it changes.
