Disease Management: Learning to Live Well with a Chronic Illness
Are you living with a chronic disease? Holy Cross Hospital can help you learn to manage your disease, become your own health advocate—and live a full and active life despite your illness.
Chronic Disease Self-Management Course –This free six-week program teaches adults with chronic medical conditions how to manage their own health. Anyone living with a long-term health condition, such as arthritis, diabetes, asthma or high blood pressure, is welcome to attend. Family members, friends, and caregivers are also welcome. The hospital developed this program with funding under the American Recovery and Reinvestment Act of 2009 and with funds granted through the Maryland Department of Aging by the Federal Administration on Aging. Diabetes Self-Management Education Class – A registered nurse and dietitian conduct our diabetes self-management classes and help you learn to manage your disease in partnership with your physician. The class includes four group sessions and one individual visit with a dietitian. View schedule and registration information.
Better Bones—This free program for people ages 55 and older focuses on ways to increase bone strength through bone-healthy exercises. The program can also help to improve balance and flexibility, and enhance your energy and well-being. View schedule and registration information.
People with Arthritis Can Exercise (PACE) (Spanish)—This exercise class, instructed in Spanish, can help people with arthritis improve their range of motion and promote healthy joints. View schedule and registration information. See Stay Active - Move! below for a similar class taught in English.
Parkinson's Exercise Class — This class is especially for seniors 55+ who have Parkinson's disease. It can help them maintain mobility, improve coordination and balance, increase muscle strength, reduce stress and improve control over movements, such as walking. Stay Active-Move! - This class helps to build strong muscle around the joints and increases flexibility and endurance. It reduces inflammation from arthritis and related conditions and reduces the risk of other chronic conditions. View schedule and registration Information. Support Groups—Holy Cross Hospital support groups are one of our most important resources for older adults who are facing an acute illness—or have a chronic health problem. The groups provide a forum in which to share medical information and resources, as well as personal stories with other people experiencing the same problem. View more information.
