Please make sure you have the following items with you at your appointment:

1. referral letter from your physician
2. your most recent radiographs/films (MRI, CT, X-ray). Please bring the actual films (not just the report) so that we can evaluate you. Without these films, we will reschedule your visit until you can obtain them. If your radiographs were done at Holy Cross Hospital, you do not need to bring a copy with you.
3. insurance information and insurance card
